Dynamic web page development with CSS and Ajax - United States

I am looking for someone with web page development experience with DHTML.

The site will get some data from the database (mysql or sql server) and show them in a tabular format. The updates will be done in ajax.

The project is in design and analysis stage, so any popular technology can be chosen for implementation (e.g. PHP)

An example application would be a banking application, where account transactions are displayed, and transfers are done from one account to the other without leaving the page.
The HTML and CSS knowledge is key. Javascript, mysql, php is required, Ajax is desired.

A previous work demonstration is required.

A mockup of the intended site is required.

A strong plus would be someone who could design a page with multiple
